Chiari malformation

It a birth defect where in part of small brain i.e. cerebellum sags down causing pressure on the brain stem resulting in collection of water inside spinal cord which is known as syrinx.

It usually presents with progressive pulsatile headache to begin with and then gradually leads to imbalance and weakness in both hands.

36 yrs lady presented with neck pain and right hand weakness.

MRI Neck showed Chiari 1 malformation causing Cervico-dorsal syrinx.

MRI showed Chiari malformation and Syrinx
Post op (6 Months) MRI showed collapse in the syrinx size

Underwent formen magnum decompression

showed exposure
Durotomy at C V Juction
Duraplasty using fascia lata
At the closure
